Abstract

This article examines systematised physical exercise as an adjunct resource in the treatment of depressive disorder, starting from a recognised difficulty of care practice: poor adherence to pharmacological treatment, determined chiefly by response latency and by adverse effects that appear before clinical benefit. The argument proceeds in three movements. The object is first delimited by distinguishing the uses of the term depression — feeling, symptom, syndrome and illness — and the criteria that support diagnosis. The limits of drug treatment are then examined, both those of a pharmacological order and those deriving from the expansion of the concept of depression over ordinary life situations. Finally, the available evidence on physical exercise and depressive symptomatology is assembled and organised across three planes of mechanism: neurochemical, psychological and social. It is argued that exercise constitutes neither an alternative to conventional treatment nor a merely hygienic accessory: it operates upon dimensions the drug does not reach — self-esteem, body image, reinsertion into social bonds — and does so at reduced cost and with a favourable adverse-effect profile. The prescribing conditions this position imposes are discussed, in particular the inadequacy of competitive modalities and of goals incompatible with the patient's clinical state.
